Daily Health Survey - Instructions

Survey Information

Survey Access Instructions:

1. Open the Firms Apps folder located on your Desktop

2. Double-Click the icon for BE Health Survey and your web browser will open

a. At this point you may need to log in before proceeding. Please follow the steps below to log into the survey

3. At the Sign in screen, even though it says Email or Phone, you need to enter your full username@bracheichler.com which is NOT the same as your email address (see example below), and then click Next

a. Example: ABC1234@bracheichler.com

4. The next page will prompt you to Sign in with your organizational account and should already be populated with your username from the previous step. Enter your password and click Sign in

5. When prompted to Stay signed in? you can click on Yes as long as you are not logging in from a shared computer.

6. You will now be brought to the Brach Eichler Daily Health Survey, answer all of the questions and then click Submit
